Terms of Service for PDS Form Filler

Welcome to PDS Form Filler (the “App”), developed and maintained by Jib Bernal (“Developer,” “we,” “us,” or “our”). By downloading, installing, or using the App, you agree to be bound by these Terms of Service (“Terms”). If you do not agree to these Terms, please do not use the App.

1. Important Disclaimer of Government Affiliation

Unofficial Utility: The App is an independent, unofficial digital form-filling tool created solely to assist users in completing the Philippine Personal Data Sheet (CS Form No. 212 Revised).
No Government Entity Representation: The App is not affiliated with, endorsed by, commissioned by, sponsored by, or in partnership with the Civil Service Commission (CSC) of the Philippines, any government office, or any government official.
Official Sources: Official blank PDS templates and guidelines may be obtained directly from the official Civil Service Commission website.

2. User Responsibilities & Accuracy of Form Data

User’s Sole Responsibility: You are solely responsible for the accuracy, authenticity, completeness, and legality of all information typed, saved, or generated within the App.
Legal Consequences of Misrepresentation: Making false statements, misrepresentations, or fraudulent declarations on an official government document (including the Personal Data Sheet and Work Experience Sheet) carries serious legal and administrative liabilities under Philippine law. The Developer assumes no responsibility or liability for any inaccurate or falsified information entered or submitted by the user.

3. Prepaid Subscriptions & Google Play Billing

Core Features Are Free: All core form-filling, local data saving, and PDF generation/export features are 100% free and do not require any payment or subscription.
Prepaid Non-Auto-Renewing Ad-Free Access:
- We offer optional prepaid ad-free passes (for example, 30 Days or 365 Days) to remove advertisements.
- No Automatic Renewal: Prepaid passes are fixed-duration access passes. They do not automatically renew or charge recurring monthly or annual fees. When your prepaid duration expires, the App seamlessly returns to the standard free mode with ads.
Payment Processing & Refunds:
- All purchases and transactions are handled directly and securely by Google Play Store via Google Play Billing.
- We do not collect, process, or store financial account or payment card details.
- Refund requests, subscription management, and payment disputes are governed by Google Play’s refund policies.

4. Local Data Storage & Backup Responsibility

Local Processing: Form entries are processed and stored locally on your device using Android encrypted preferences.
Backup Responsibility: Because we do not maintain a central database server with your personal form entries, you are responsible for maintaining your own backup files or exported PDF copies. Uninstalling the App or clearing app data will remove locally stored entries.

5. License & Intellectual Property

Limited License: We grant you a personal, non-exclusive, non-transferable, revocable license to use the App for your personal form-filling needs.
Restrictions: You agree not to reverse engineer, decompile, modify, redistribute, or create derivative works based on the App’s source code.

6. Limitation of Liability & No Warranties

“AS IS” Service: The App is provided on an “AS IS” and “AS AVAILABLE” basis without warranties of any kind, either express or implied.
Limitation: To the fullest extent permitted by applicable law, the Developer shall not be liable for any direct, indirect, incidental, or consequential damages resulting from the use or inability to use the App, data loss, or rejection of generated PDF documents by receiving agencies due to user input errors or printing variations.

7. Changes to Terms

We reserve the right to update these Terms from time to time. Any changes will be posted on this page with an updated “Last updated” date.
